A dental implants is a screw, usually in titanium, implanted into the jawbone on which a dental crown, bridge or removable denture can be fixed. Once it has been screwed into the jaw, a process bonding the bone and the implant takes place, it is called osseointegration. This process can take from 1 to 4 months, according to the implant and the quality of the patient’s bone. It is generally recommended that you wait until the end of this period before fixing a tooth, bridge or denture on the implant. An implant abutment is screwed to the implant and the denture is either cemented or screwed (which is referred to as a cemented crown or a screw-retained crown).

Dental implants are the most advanced solution available today to replace missing teeth.

Guided Surgery

When placing a dental implant, the oral surgeon clears the gum tissue down to the bone to target the exact location of the implant.  Then after the insertion, he sutures up the open gum tissue to begin the healing process. Computer-assisted surgery enables measurements with maximum precision and determines the virtual position of implants before placing them in order to achieve better functional and aesthetic results.

The process of guided surgery begins with the combination of images from an intraoral and cranial scanner, through which an exact three-dimensional model is created that maps down the mouth to millimetre. Once the 3D model is obtained, the surgeon can simulate the operation in the computer and place the implants in the optimal position to achieve perfect aesthetics and function.
Once these measurements are taken, a surgical guide is created by the laboratory, which reproduces the position of the implants with utmost precision. These guides enable the implants to be inserted by simply drilling them to the pre-established depth without opening the gum first, thanks to a series of holes located in the exact position where they are to be placed. This allows for a minimally invasive surgery, without lifting the gum tissue and without placing stitches in most cases. In addition to all that, surgery time is considerably reduced.
Less invasive and shorter surgery results in a more comfortable postoperative period for the patient without incising, i.e., stitches.

Bone Loss- The solutions

Bone Augmentation / Bone Graft

In certain cases when the patient does not have sufficient bone, implants cannot be inserted straight away. If one or more missing teeth are not replaced quickly, it can result in the deterioration and resorption of the jaw bone. Sometimes it only takes a year to lose a significant amount of bone. To avoid bone loss, it is recommended to replace the lost natural tooth with a dental implant as soon as possible. Other reasons can also be responsible for bone loss, such as gum disease, periodontitis, infections and other conditions of the jaw.

Bone augmentation, also known as bone grafting, is a procedure typically needed when the current bone level in the jawbone is not sufficient for integrating the dental implants successfully. Fortunately, our surgeons are able to carry out bone augmentation procedures when necessary. When the bone mass is too thin to hold an implant in place, bone augmentation will help the bone to fuse with the dental implant and hold it securely. If a dental implant is placed in a jawbone where the bone structure is insufficient, it is more likely that the implant will fail.

Sinus Lift

A sinus lift is a procedure when the oral surgeon increases the amount of bone in the upper jaw. If you need dental implants but lack support in your jawbone, you will more than likely need a sinus lift. The sinus needs be moved to make room for the inserted material (artificial bone or bio-bone). Thus, the surgeon incises the gum, lifts the sinus and inserts the bio-bone which will play the role of bone.

Implant crowns and bridges

The implant crown, or bridge is like a conventional crown or bridge, except that the base is not a natural tooth but an implant, so that the bite is supported by the implanted artificial root. Basically, at the level of its fixing mechanism, we can distinguish two types of connections between the implant and the crown / bridge in progress; we talk about either a cemented solution or a screw-retained solution:
Cemented crown / bridge
As the name suggests, this involves cementing the crown to the implant. In the front, we recommend that you choose a metal-free solution to achieve maximum aesthetics. In the case of cemented work, the subsequent correction of the prosthesis is quite difficult if it turns out to be necessary, since in this case the titanium abutment is fixed to the implant, and then the implant crown or bridge is glued to the abutment.
Screw-retained crown / bridge
In the case of a screw-retained crown or bridge, the finished work is fixed to the implant by a screw that goes across the crown. After stabilization of the crown, the screw thread is covered with a special material.
A great advantage of screw-retained solutions is that, if necessary, they can be easily removed without damaging the ceramic, and then replaced after the necessary correction. In the case of bonded solutions, the removal of the prosthetics will damage them. Luckily, it is exceedingly rare that a prosthetic work needs to be modified.

Implant denture

Removable dentures are unpleasant because they move while talking, eating, or laughing, causing unpleasant situations to the wearer. However, this can be easily avoided with a simple procedure. All we must do is use dental implants and put attachment points (locators) on the denture to keep it from moving.
In the absence of teeth, traditional dentures only sit on the gingiva, so they may not be stable enough, wobble during meals or speaking, slide away from the membranes, which can only be held in place by using a denture adhesive.
In case of total absence of teeth, by using implants, we can increase the stability of the denture, thus increasing your comfort and regaining its chewing function. Implant-fixed dentures can be used when the patient has lost all the teeth of a given jaw and the required size and number of implants can be implanted into the jawbone, provided there is sufficient bone structure.

Counter-indications for dental implants

The decision to fit a dental implant can depend on the general health of the patient, the local health of the surrounding tissue and of the jaw, as well as the shape, size and position of the bones of the jaw, adjacent and opposing teeth. Health problems prohibiting the fixing of implants are rare, even if some factors can increase the risk of failure. Patients with bad mouth hygiene, heavy smokers and diabetics run the greatest risk of contracting some gum diseases, which increase the long-term failure rate. The long-term use of steroids, osteoporosis and other bone diseases can also increase the risk of failure of good osseointegration of the dental implants.
